A nursing care plan is a formal document that outlines a patient’s needs and requirements and is created in collaboration between the patient and a clinician. It can look very different from individual to individual as it is highly personalised to support their specific conditions, needs, preferences etc and it typically includes things like medicine and level of care needed, what are the treatment goals and any other details that may be relevant.
