Whilst we all hope that moving into a care home is a perfect fit, sometimes circumstances may mean that moving from one home to another needs to happen. Moving From One Care Home to Another

As with moving into a care home for the first time, making the right choice for your new care home is extremely important. Even if you have done all the right things when choosing a care home the first time around such as visiting the home, asking all of the right questions and meeting staff and residents, sometimes things just aren’t a right fit.   • - Ensure your new care home can cater for your needs. If your current care home no longer fulfils your needs, you can request a needs assessment from your local social services, they can then help advise based on your new needs.

  • - Review your current care home contract. If you are a self-funder it is important you review the terms and conditions of the cancellation process including the required notice period. If your care has been organised through your local authority, call your local council to see how they can help you move care home.

  • - Visit your new care home to get a feel for the environment and how it works to make sure it is the right fit for you.

  • - Make sure family and friends are aware of the move and the new address.

  • - Create an itemised list of everything that should be moving with you or your loved one.
